Community collaborators

CentraCare - Long Prairie Clinic provides a variety of visiting specialty services, onsite lab and imaging services, in addition to physical, occupational and speech therapy services.  

This pilot gives us a chance to better serve our patients outside clinic walls. With stable, high-speed internet at home, more residents can attend virtual appointments, manage chronic conditions remotely, and access health education tools that support their well-being.
